There are so many interesting things going on here it's difficult to pick a starting point. The most powerful aspect of this image is the, seemingly, artificial nose. The harsh line leading to it is a strong indicator of separation, and the tone and colouring of the nose itself shows it is somehow other than the character it is attached to, which, of course is interesting given the connection of our sense of taste and smell, and the fact that this creature has 3 mouths. It is almost as if saying, no matter what he consumes he will never be satisfied, and at the same time it's commenting not only on our insatiable appetite but the artificial, or at least misplaced, character of our appetite. Makes the viewer wonder, maybe, if it's not the case that our appetites could be satiated elsewhere, in different manners than those we have currently dedicated space to.
All of this is further complicated by the tiny beings crawling all over this unnecessarily armed devouring machine, as well as biting him. Tiny beings who could not possibly be effected as detrimentally should their "desires" go un-met as their host is by just that. His eye, focused yet wandering, has both given up and claimed he will never give up, no matter how many mouths, how many parasites show up to devour him as fast as he devours the world.
The colouring and harsh texture and lining that Barker uses here, as well as with most of his work, gives a simple, complicated, brutal and human feel to the mystical images his imagination comes up with. It gives us the sorts of nightmares we can't help but think about, and desire to get back to.
